The following form should be completed in its entirety. This form will then be transmitted to the Worship Ministry Department. Once the form is reviewed, a permission form will be emailed or mailed to you. Please fill out and return to the Worship Department to complete the registration.

PLEASE READ THE FOLLOWING CAREFULLY

Duration:
Lessons will last thirty minutes. The instructor may pray with the student before and after each lesson.

Payments:
Monthly payments will be made directly to your instructor, and will be due at the first lesson of each month. Non-payment after a one-week grace period will result in termination of lessons. At time of payment, we request two checks: one check will cover the teachers 10% advertising fee-this check will be made out to Harvester Christian Church/Harvester Music Academy and the remainder (90%) will be made out to your instructor.

Location:
Classes will be held in the Family Life Center in assigned Music Academy classrooms on the second level, Tuesdays and Thursdays. HCC strongly discourages lessons at times other than Tuesdays and Thursdays for a number of reasons so we request that lesson times be scheduled ONLY on Tuesdays and Thursdays.

Late Arrival:
Late arrival will cut into the student’s lesson time. The individual instructor will handle make-ups, cancellations and rescheduling of lessons. Students should arrive 5-10 minutes prior to lesson time. In the event you are unable to make a lesson, please contact your instructor directly.

Emergencies:
In the event of an emergency, I authorize first aid or medical treatment for this student and release Harvester Christian Church from any and all responsibility in connection therewith.
